Bug Description
seeing that the toc work in calibre editor but not in viewer in tried to check in epubcheck. i then found 2 errors in metadata.opf :
- no attribute version="2.0" for package tag
- no attribute toc="ncx" fon spine tag
after those changes, epub worked fine

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
