Headers and footers don't appear EPUB to PDF

Since last 2.0 version, whenever I convert EPUB files to PDF, this header template doesn't work anymore:

<div style="font-size:x-small"><p style="float:left">_TITLE_</p><p style="float:right;"><i>_AUTHOR_</i></p></div>

I always put that command in Output Options / PDF Output / Page headers and footers

"Add page numbers to the bottom of every page" option in PDF Output tab doesn't work either.

Headers and footers don't appear in converted PDF file.

I have enough space in margins (top and bottom): 60.0 pt

My system is Windows 7 Home Premium 64 bits

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
