display plugboards with disabled plugin

Bug Description

As discusseed here: http://www.mobileread.com/forums/showthread.php?t=181654 when a metadata writer plugin is disabled, the format disappears from the plugboard (existing plugboards are hidden, and the format cannot be selected for new ones).

Although this makes sense, it can be confusing. It can give the impression that no plugboards are defined, and if the plugin is enabled again the user is not immediately aware that the plugboards are active. Besides, the plugboards could be used for other purposes (the Kobo Utilities plugin, for instance, uses them to update metadata in the device database), and one would expect the plugboards to be visible and usable even if the writer plugin is disabled.

Maybe there could a checkbox to show/hide plugboards for disabled plugins, or they could be greyed-out or starred or something, in case simply showing all plugboards is not desired.

Although I am not completely convinced that allowing plugboards for formats that have no writer, I implemented this enhancement.
