convert html file encoded as utf-16 fail to include image files and have wrong order

Bug Description

 The calibre version : 1.19.0
the operating system you are running calibre on: windows 7 64 bit

in my include zip file, there are 2 folders with exact same files, but one is encoded as utf-8, the other is encoded as utf-16.
I just add TOC.html (each folder) to calibre, convert it to epub, and save (<test folder>/unknown/test/test - unknown.epub.

if you compare the epub in utf-8 and utf-16 folder, 2 bugs can be seen:

1. there is no images included in utf-16, images can't be displayed.
2. the link sequence in utf-8 (as in TOC.html) is preface.html, chapter01.html, however, in utf-16's epub, when you view pages one by one, chapter01 is displayed first, followed by preface

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
