[Enhancement]ToC Editor senses when link is to top of file

As per our conversation on MobileRead (
http://www.mobileread.com/forums/showthread.php?p=2712866#post2712866) I am
adding this request to make it easier to track:

When using the ToC editor and generating listings from major/all headings,
if the heading is the first element on the page, it would be nice to link
directly to the page, rather than adding a UUID to the heading and then
linking to the UUID. The end result is the same and this would be cleaner.

Whenever this gets around to the top of the deluge, :) this would be great.

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
