txt → epub When I don't give a title, the <title> tag is set to unknown. It should default to the chapter name and chapter ID for each fragment. Or use the name of the converted file

Bug Description
Hi,
In the generated HTML files inside the resulting epub file, the <title> tag is "unknown" instead of the book's title. Mostly because I'm lazy and didn't specify it anywhere. But...
When in doubt, calibre should use the name of the file the user selected for conversion.
And as an extra, it could also add the chapter ID and its detected title before it (or after) said file name.
Anything is better than seeing "unknown" all over the place in my reader
