Bug Description
Error occurred for Calibre version 0.9.20 & 0.9.21 (Windows 64 bit) - it works for version 0.9.19 and below
If the file in Haodoo's PDB or UPDB format is converted to EPUB, the page break is inserted after the page heading. There is no such error if converted to MOBI format.
My Calibre setting:
Look & Feel: Remove spacings between paragraph on
Heuristic processing: enabled
Page setup: Output profile is Kindle
others are default settings

Fixed in branch lp:calibre. The fix will be in the next release. calibre is usually released every Friday.
