bzr add should return an error if the file should be ignored

Bug Description
We have in the last time the problem that a file would be added, which is located in a folder which should be ignored. The add command came from qbzr-eclispe. I think it would be useful to reject the add and force the user to use bzr add --force to add such a file.

I think we previously had this behaviour and people didn't like it. We could possibly add an option to warn or refuse it.
