bzr add should return an error if the file should be ignored

We have in the last time the problem that a file would be added, which is located in a folder which should be ignored. The add command came from qbzr-eclispe. I think it would be useful to reject the add and force the user to use bzr add --force to add such a file.

I think we previously had this behaviour and people didn't like it. We could possibly add an option to warn or refuse it.

I'm marking this easy because it should be fairly small to add, but it may need some further discussion about just how to do it. If anyone's interested in adding this, just ask.
