support bzr-over-ssh-over-http(s)

Bug Description

It will be used via firewall, proxy or TOR.
If use proxytunnel [1] or httptunnel [2], it may work. But native support is good for Windows and lightweight bzr users.

If you can connect via 'ssh' to a port, then you can just use:

bzr+ssh://host:custom_port/...

Certainly one could provide a plugin to give bzr+ssh+http if it requires custom requests from the client. I doubt we'd include that in core before it showed itself useful to many people, though.
