Adding certain icons from unity lense to unity launcher does not show

Bug Description
I installed Anjuta yesterday, and when I tried adding it the the launcher today it added it but no icon is present (see screenshot). Although the icon is not present it still launches the application. I am unable to get a screenshot when the context menu is pulled up, but when you do right click on the icon, normally the name of the application would be shown, but in this case it is empty.
This application was pulled from the Recent Apps list. I just tried adding GIMP to the menu and I get the same result as above when taking it from Recent Apps. I also tried Disk Utility by searching for it, launching the app, closing the app then dragging and dropping it on the unity launcher, and that worked ok.

I've seen this problem with 11.10 as well and if you restart your computer the icon shows up.
