Hello
Start a live session of today's amd64 build and run sudo apt-get update, it will return:

W: Duplicate sources.list entry cdrom://Ubuntu 12.04 LTS _Precise Pangolin_ - Beta amd64 (20120406)/ precise/main i386 Packages (/var/lib/apt/lists/Ubuntu%2012.04%20LTS%20%5fPrecise%20Pangolin%5f%20-%20Beta%20amd64%20(20120406)_dists_precise_main_binary-i386_Packages)
W: Duplicate sources.list entry cdrom://Ubuntu 12.04 LTS _Precise Pangolin_ - Beta amd64 (20120406)/ precise/restricted i386 Packages (/var/lib/apt/lists/Ubuntu%2012.04%20LTS%20%5fPrecise%20Pangolin%5f%20-%20Beta%20amd64%20(20120406)_dists_precise_restricted_binary-i386_Packages)

I have almost the same issue, but not in a Live Session. I upgraded to 12.04 LTS via 11.10 and 12.04 Beta2.
Now I always receive a "Duplicate sources.list entry" error whenever I apt-get update. (I update daily.)
I'm on an amd64 machine and don't know where the i386_Packages file in /var/lib/apt/lists came from. (There is no corresponding entry in sources.list)

I tried deleting the i386_Packages file from /var/lib/apt/lists and it solved the issue until the next reboot, when it magically reappeared in

oops.. "... reappeared in /var/lib/apt/lists. Btw, I no longer have a prerequists-sources.list in /etc/apt/sources.list.d/

cjwatson asked to file the bug against apt, although the actual fix is probably in livecd or something like that.
