Activity log doesn't show who added a task or when

Bug Description

When someone chooses one of the "Also affects:" options for (i.e. adds a task to) a bug report, this is not recorded in the activity log.

For example, <https://launchpad.net/ubuntu/+bug/1/+activity> does not show who mistakenly targeted the bug at ichthux, rosetta, Baltix, Tilix, bum, or Ubuntu Firefox, or when any of these things happened.

There are `record_bug_task_added` and `record_bug_task_edited` functions in subscriptions/bugactivity.py, and they're configured in bugactivity-subscriptions.zcml, which points to the possibility that events are not being generated at the time that tasks are created or edited.
