Sound Settings icon several times in launcher

I can reproduce this fairly easily with the recent unity versions (5.4).

See the screenshot -- the sound settings icon is 2x in the launcher (but I've seen it also 3x) even though only one instance of sound settings exists.

I'm not exactly sure how to reproduce this step by step as the 2nd icon appears only after some time of use. I have an USB headset so I'm often going to the sound indicator and clicking on Sound Setting when I need to call somebody (I change the Input/Ouput from Analog to the USB headset and back). I do this several times a day. Sooner or later the 2nd icon always appears.

this is probably bug 928912 you facing. there is a test case in that report to reproduce the bug as well.
