Single windows show up twice in launcher and Alt+Tab

Occasionally, when launching an application, the launcher shows two windows being open instead of one. Clicking on the icon when focused still shows only one window to pick. However, when using Alt+Tab, it shows the same window duplicated twice in the preview.
