LibreOffice doesn't have window title or buttons in Unity

Bug Description

LibreOffice (Writer and Impress) shows no window title or buttons or anything in Unity when maximised. It also doesn't appear in the Unity panel while open.

This does not happen every time the program is launched. It seems that this occurs when directly opening a file (e.g. double clicking a file in Nautilus or downloading a .ppt or .doc in Firefox and clicking "Open" instead of "Save")

Unmaximising shows the window title and buttons, but the icon still doesn't appear in the Unity panel.

Seems to be initiated when jumping between full screen apps, particularly to/from nautilus. When it happens, Libreoffice also disappears from the alt-tab list and, if minimised, also from the expo (ctrl-w) view. So it completely disappears from view and access from Unity when minimised. However, if you close all other apps then start a new app then Libreoffice springs back into view.

I originally had this problem with Libreoffice 1:3.4.5 from the ppa so I purged it back to the standard 1:3.4.4 packages but find the problem still exists.
