USB disconnect after while (ehci_hcd, uhci_hcd)

Bug Description
When I plug in card reader to usb (Axago All in One) it gets mount correctly:

But after while (sometimes less than minute, sometimes longer):

device is disconnected and detected as full speed USB using uhci_hcd (previously detected as high speed USB using ehci_hcd).
And then it is unable to enumerate. No /dev/sdb1 device is then accessible.
It's annoying especially if you want quickly move photos from card and go away for another shots.
Please explain where the bug is or release fix.
