Bug Description

step by step:
1. I started gnome-terminal from unity panel (launcher).
2. I closed gnome-terminal
3. I started gnome-terminal from unity panel (launcher).
4. I closed gnome-terminal
5. I started gnome-terminal from unity panel (launcher).
6. I closed gnome-terminal
There was three or four icons for gnome-terminal at panel (despite only one process gnome-terminal running in the system) after all.
Then something magic happens and three "fantom" icons disappeared^W became invisible, but functional yet.
Then I started gnome-system-monitor and three icons for system monitor appears simultaneously (yes, for only one process gnome-system-monitor, see screenshot).

this happen to me with the gnome-system-monitor icon in Ubuntu 12.04. I searched the applicaction and do drag-drop the icon to the launcher.
Running unity --reset solve the problem.
