Bug Description
Version 0.8.34 in WindowsXP SP3
Converting a very clean HTML file with 100 chapters to epub, mobi and lit. TOC generator stops TOC at 50 entries in all 3 formats.
I've attached the HTML file and the cover. calibre converts the HTML just fine. The TOC just stops at 50 entries.

There is an option in the conversion settings to raise that limit.
