Bug Description

User story:

As a SSO user, I want to add a 2-f auth device to my account so that I can register my own 2nd factor device.

Details:

Allow an user to add multiple 2-factor auth devices via his account details page.

Related work include:

- allow the user to choose from different types of 2-f devices (Google Authenticator, YubiKey, generic HOTP)
- include device-specific instructions
- customize the registration flow based on the device requirements (eg, provide QR code images for Google Authenticator)
