some windows lack title bar and buttons when maximized, and are not displayed in launcher

Bug Description

Steps to reproduce:
- browse local folders in Nautilus and look for an xls file
- double click the file so it opens in LibreOffice Calc

The attached screenshot shows the LibreOffice Calc window maximized.

Usually in Unity, by moving the mouse to the top of the screen, the application's menu AND the window's control buttons (minimize, unmaximize, close) should appear.

In this case, instead, whether the mouse is on the top of the screen or not, the application's menu is always shown and is _below_ the window's top bar (which I don't dislike but is inconsistent with the usual behavior) and, much worse, the window controls will never show up. Also, clicking and dragging the window bar won't unmaximize it. There is no way to unmaximize the window, and the only way to close it is to use the File/Quit menu

And even worse than worse, no icon is shown in Launcher.

This does NOT happen if I open LibreOffice Calc from Dash. This only happens if I open a file by double clicking it in Nautilus.

Up to now I've only seen this with LibreOffice, so this may be related to an issue in LibreOffice itself; however the very fact that this even CAN happen is in itself an issue in Unity, not to mention LibreOffice doesn't have to be aware of Unity and it doesn't show any similar problem in Gnome.
