Bug Description
My computer had previously had windows xp installed on it, but when it would not boot under any of the possible choices, I decided to install Ubuntu via a USB storage device(4 GB). When I started the Ubuntu installation I completed all of the pre-intsallation information. I then started the installation process; when the progress bar was about 40% done it stated that it was "creating user. . ." and then the error window popped up saying that the installation has crashed. So then i proceeded to creates this error report

Reviewing your log files attached to this bug report it seems that there is a problem with the disk to which you are trying to install Ubuntu. I'd recommend performing a back up of your existing operating system and then investigating the situation. Measures you might take include checking cable connections for your disks and using software tools to investigate the health of your hardware.
