Bug Description
When an applicant is hired, he is recorded as an employee. Then, in the applicant form, it is possible to click the button reset to new.
If the button hire is used with the same applicant form, a second employee record is created for the employee.

The system already asks if you want to create a new employee, so if the users clicks yes, it is normal that a second employee is created with the same name. The user should click "no" when doing this, a second time.
