Window buttons doesn't appear when opening a file

Bug Description
When opening a file from a file manager (ex. double-clicking on a Writer document in Nautilus), sometimes (not always) the window buttons (close, minimize and maximize) doesn't appear, but the global menu still works.
(Also the button on the Unity Launcher - I'm using Ubuntu - doesn't appear, but I think this is another bug in another package).
Steps to repeat:
1. Open any LibreOffice document by double-clicking it.
2. Put the cursor on the global menu bar. The menu shows, but the window buttons doesn't.
I don't know if this is a bug in lo-menubar, or in Unity, or in LibreOffice.
I'm using Ubuntu Oneiric (11.10), on a Core 2 Duo E7400, 4GB RAM, nVidia GPU 9400GT.

The icon not appearing in the launcher seems to be a related issue, not a separate one. On my system, if I use the global menu extension, the menu bar, title, window buttons and launcher icon all fail to appear. The absence of the menu bar effectively makes the application unusable. If I remove the global menu extension, the menu bar appears beneath the panel because the global menu functionality isn't there, but the title, window buttons and launcher icon fail appear.
