I guess the problem would have vanished after a reboot. Also zeitgeist might have crashed for you, I have seen a similar things in apps lens a few days ago.

The problem continues to occur after restarting and running all the latest updates. Sometimes files are displayed, 9 out of 10 times they're not.

Thanks for the bug report. Is that still an issue for you Ubuntu 12.04 with all updates applied? Can you please test that and let us know?

I have this issue on Ubuntu 13.10, with latest updates installed (and some PPAs: one for rhythmbox, oibaf mesa PPA, steam commercial repo, ubuntu sdk team, ubuntu touch coreapps drivers, webupd8team java). It started a few weeks ago and really kills my usual workflow.

I tried:
rm -r ~/.local/share/zeitgeist (Don't care about history) and rebooting
Reinstalling the files lens
Reinstalling zeitgeist-datahub
zeitgeist-daemon --replace (this says "Unable to get name "org.gnome.zeitgeist.datahub" on the bus!" and then doesn't return to shell, is that supposed to happen? Always reads like it should return to shell..)

Some info:
$ ps aux | grep zeitgeist
flo 2678 0.0 0.1 494404 8308 ? Sl 11:44 0:00 zeitgeist-datahub
flo 3365 0.0 0.0 340040 4388 pts/1 Sl+ 11:46 0:00 zeitgeist-daemon --replace
flo 3371 0.0 0.1 238816 10136 ? Sl 11:46 0:00 /usr/lib/x86_64-linux-gnu/zeitgeist-fts
flo 3739 0.0 0.0 10984 908 pts/7 S+ 11:56 0:00 grep --color=auto zeitgeist

$ ps aux | grep lens
flo 3649 0.0 0.1 330004 9528 ? Sl 11:53 0:00 /usr/lib/x86_64-linux-gnu/unity-lens-music/unity-music-daemon
flo 3741 0.0 0.0 10980 900 pts/7 S+ 11:58 0:00 grep --color=auto lens

Manually starting /usr/lib/x86_64-linux-gnu/unity-lens-files/unity-files-daemon does not fix my issue.

Any other info that might be relevant?

I finally managed to fix my configuration.

In dconf, I had com/canonical/unity/lenses/disabled-scopes set to ['files-local.scope']. Resetting this to the default [] value and restarting unity fixed my files lense. FINALLY, YAY! :-)

I *really* wonder how that value sneaked in there. I don't remember changing it, simply wondering one day why my files lense suddenly stopped working.
