"Also affects distribution" fields default to the worst possible values

Bug Description

1. On a bug report marked as affecting one Ubuntu package, choose "Also affects distribution".

What you see: The "Distribution" field defaults to Ubuntu, and the "Source Package Name" field defaults to ... the package on which it is already filed.

In other words, Launchpad defaults to the only combination that it can be absolutely 100% sure that you *don't* want to choose.

"Also affects distribution" is most often used not to say that the bug affects another distribution, but to say that it affects (or might be present in) another package in the same distribution. Therefore, an appropriate solution would be to default the "Source Package Name" field to being completely blank.

A bonus would be to pre-fill the currently filed source package name *only if* you select something different in the "Distribution" field (and didn't previously edit the package field).
