homogenize VTK output so that geometry of events match event index

Paraview display a fault geometry (rectangular patch) assuming incorrectly that the fault number corresponds to a time index. A separate series of files should be created with the fault index tied to the current output number to facilitate the generation of animations where the faults appear well synchronized with the displacement field.
