Please remove the model=test parameter, and see
https://wiki.ubuntu.com/Audio/InstallingLinuxAlsaDriverModules.

I removed the mode=test parameter and installed the new alsa driver from PPA, but I still have the same issue.

Something is amiss: you appear to be using the old driver still
(1.0.21), whereas a newer one (2.6.32-33.201108021603) has been
available for ~20 hours. What is the output in a Terminal/Konsole
from:

modinfo snd-hda-intel

I apologize. I had a finger check and installed a package with the wrong kernel number.

After installing this, however, now I don't get sound from either the headphone jack or the internal speakers.

I have attached the latest modinfo snd-hda-intel.

Thanks for your help.

what are the default and current in-use sinks?
