login text fields need placeholder text

Bug Description
The login fields in both the dropdown and the login page need placeholder text that says "User Name" and "Password" or whatever is appropriate. Currently they're blank with no titles and it's unclear what they're for.
We could simply use HTML5 placeholder text to cover a large portion of browsers, or we can do whatever we did with the search field.
