Bug Description
A proxy may allow access without authentication after requiring NTLM before (e.g. it may cache authenticated IPs). In such case Polygraph receives 200 (OK) reply instead of NTLM authentication response. This results in "proxy authentication failed: access allowed with invalid credentials" error.
Polygraph should not require a proxy to do NTLM authentication for a new connection if the proxy did it before.
