Bug Description
I downloaded as setup revision 4662 today and found that head.php missing from the default Vesuvius theme. Copying from the lpf3 theme folder provided a workaround.

The theme has changed significantly since 0.9.0.... the 0.9.1 vesuvius theme will be rebuilt from the 0.9.0 theme before packaging.
