this issue hit me after upgrading a freshly installed 12.04 LTS -> 14.04 LTS.
since the system was unconfigured yet, i allowed overwriting all config files with new defaults from new packages.

a fresh install of 14.04 LTS did not show these symptoms.

This release of Ubuntu is no longer receiving maintenance updates. If this is still an issue on a maintained version of Ubuntu please let us know.
