"Manage Members" view does not prefill moderator checkboxes

Bug Description
Noticed this during testing. On staging, go to:

None of the moderator checkboxes will be checked. Submitting the form as is will cause a validation error: "Must leave at least one moderator for community." No way to know who was a moderator previously.

Form actually seems to be working correctly. I think I got psyched out because Paul had previously been the sole moderator of the "Paul Test Tries" community but was then removed by GSA. This left the test community in a state where it had no moderator, but the form works fine once you use it to set one or more moderators.
