Binary package hint: unity
Clean install of Unity inside VirtualBox. The theme seems to work then crash very shortly after logging in. See screenshot to see how the theme looks (grey panel and 'old' icons).

Alan, this is a crash in gnome-settings- daemon, not unity (see apport color as well). Can you report all crashes with the .crash file you can fin in /var/crash/*.crash (you should have a gnome-settings- daemon one) so that we are able to debug it? Without those information, there is no way to know what happened unfortunately.
