Thumbnails are all from first title

When the title_set has more than one title, all the thumbnails will be taken from within the first title. TV serials with several episodes per DVD are typically set up this way.

This is a bug in the GStreamer dvdreadsrc element. I submitted a patch to GStreamer that fixes this, but I've since started work on a complete rewrite of dvdreadsrc (well, an element to replace it). My new element is now mostly usable, but it still needs quite a bit of work.
