Bug Description
When adding an ID for a person, you can reuse the same ID across multiple people as well as assign the same ID type multiple times.

I added a unique check to the id number so it must be unique based on the id_type across all records. I also made it so that the same person can't have the same id_type multiple times. This may need to be customized for country specific implementations depending on the ID system the country uses.
