Bug Description

I need to add a HACKING file that lays out the license that contributors are agreeing to when they send us patches.

It would be also great to provide a license information comment block. In particular, we should append such a comment block to each *cpp and *.h file.

I actually think we shouldn't add license information to every file. The license just adds noise to the files. It's common practice (and I believe, legally acceptable -- IANAL) that if the project has a LICENSE file in its root, then it is assumed that every file within the tree is covered under that license.
