xorg should always probe modes on an output that has been enabled in xorg.conf

Bug Description

When xorg detects an output as 'disconnected' it will not try to probe for display modes, even if they are explicitly enabled in xorg.conf. This will cause xorg startup to fail in cases where the display device cannot properly be detected on startup or where the user wants to force a display mode on a headless configuration.
