Missing include in exceptions.h

Bug Description

I'm using clang++ to compile a client using MySQL++ 3.0.9 and is failing with error: no type named 'type_info' in namespace 'std' at exceptions.h:208 (and other places using type_info).

This can be easily fixed by adding #include <typeinfo> at the begining of the file.
