Binary package hint: indicator-session
This is a just updated Maverick, although I've seen it quite a few times under Lucid as well.
As you can see on the attached screenshot, the logout icon does not show up in the indicator applet. Instead, it seems to show my username. You can't click it - nothing happens. You can reboot by pressing Ctrl-Alt-Del and then select the appropriate item, but there is no (non-terminal) way AFAIK to log out or switch user.

An interesting notice is that the indicator reappears if you move the entire panel to another part of the screen. You can then move it back again, so that is a simple workaround.
