Delete All button breaks messages page

The presence of the 'Delete all notifications' button in /account/activity/index.php seems to break various actions on the page.

If the button is present then:
* clicking the button results in it being replaced with the list of messages and the messages not being removed from the correct listing. The messages are not actually deleted; and
* deleting an individual message results in the delete all button being replaced with the list of all messages. The message is deleted but this is not reflected correctly on the page.

Additionally, Packed.js reports an error on page load. Removing the button completely results in the page working as expected.

I've had a go at debugging this but can't work out what the problem is so far.

Tested in Google Chrome and Firefox under Debian.

This was broken due to some theming changes that moved the button inside the main form on the page. Evonne Cheung will fix it in the next round of theming.
