while reassigning and commenting on that bug, the comment has been lost.

In which textarea box did you write the comment? In the one near the
status change controls (Comment on this change (optional)) or the one
at the bottom of the page?

One option I suggested to Brad on IRC would be to only have a single form on the bug page, that includes both the comment form at the bottom plus the per-bug task widgets at the top.

With the new formlib stuff we've got, it should be pretty easy to handle them all by deriving a field prefix from the task name.

If we did this, it would probably be worth removing the comment field from the bug task widgets at the top.

It doesn't sound like this is related to Fabio's problem, based on
his followup. (FWIW, I'm not a big fan of that approach anyway,
because having to scroll all the way to the bottom of the page to
make a change comment seems entirely non-obvious to me.)

Fabio, you're saying then that when you enter a "Comment on change
(optional)" as part of the same form where you edit status, assignee,
etc. that comment is getting lost? Has it happened again before or
since?

Yes that is correct. It might have happened. I don't usually re-read bugs, but waits for emails from the system and i filter out my own changes/comments.

I didn't see it happening again.. how.. just adding comments to bugs? No seriously.. i have no idea what triggered it or how to reproduce it.

I was able to reproduce this on launchpad.dev doing the following
1. http://launchpad.dev/firefox/+bug/1/+editstatus
2. Enter any comment in 'Comment on this change'
3. Re-assign the bug to a non-existent person: 'fooobarino'
4. That will trigger a form error and the comment is gone.

I'm not sure if this is the issue Fabio originally reported.
