It seems this was accidentally tagged regression-proposed. As there haven't been any Lucid kernels in -proposed up to this point I'm removing the tag. The bug description confirms this was against the 2.6.32-20.30 kernel which was a regular lucid kernel release. Thanks.

Hi Glenn,

This bug was reported a while ago and there hasn't been any activity in it recently. We were wondering if this is still an issue? Can you try with the latest development release of Ubuntu? ISO CD images are available from http://cdimage.ubuntu.com/releases/ .

If it remains an issue, could you run the following command from a Terminal (Applications->Accessories->Terminal). It will automatically gather and attach updated debug information to this report.

apport-collect -p linux 563481

Also, if you could test the latest upstream kernel available that would be great. It will allow additional upstream developers to examine the issue. Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Once you've tested the upstream kernel, please remove the 'needs-upstream-testing' tag. This can be done by clicking on the yellow pencil icon next to the tag located at the bottom of the bug description and deleting the 'needs-upstream-testing' text. Please let us know your results.

Thanks in advance.

I'm also experiencing this issue: about once a month the entire X session will lock; even the cursor stops moving. I previously assumed that this was the hardware locking up, but I've just discovered I can SSH into the box while it's locked up. Running 'reboot' does very little, and I get a black square with some randomly-placed coloured pixels up the top-left of my desktop.

Until recently I was running Jaunty, and I was experiencing the issue there as well. I dist-upgraded to Lucid via Karmic a few weeks ago. I haven't been able to link the crashes to any specific activity. I'm reasonably sure my memory is ok; I plan to run memtest86 soon to confirm.

I'll run apport-collect as requested as soon as I can work out how to login to launchpad (#556927, awesome...).

Same problem here.

After several reports of "kernel panic" at booting time, now this is appearing:

switching to software fbcon

...
root/sys failed
root/proc failed
...
doesn't have /sbin/init

After I turn it off by force and turn it on again:

--------------------------------------------------------
Kernel panic -not syncing
....
panic + 0*48/0*f3
......
segmentation fault.... error 4 in libc.so
...
--------------------------------------------------------------------------
Then I tunned off and turn on again: the ubuntu initial scree apeared by became freeze

Turn offf and on again: it works (I using it)

This happened yesterday also.

Any idea?

Do you see messages in kern.log like

Your BIOS doesn't leave a aperture memory hole
Please enable the IOMMU option in the BIOS setup

Either go into the BIOS setup and enable the IOMMU or if your system does not have an IOMMU (too old) try editing /etc/default/grub

adding "iommu=soft swiotlb=131072" to GRUB_CMDLINE_LINUX=""

In my case the diff was

-GRUB_CMDLINE_LINUX="console=ttyS0,9600n8 console=tty0 nomodeset noacpi nosplash nofb"
+GRUB_CMDLINE_LINUX="console=ttyS0,9600n8 console=tty0 nomodeset noacpi nosplash nofb iommu=soft swiotlb=131072"

for an HP Proliant DL145 G2 server

Then run "update-grub" and reboot.

Rebuilt server using xfs instead of ext4 and still get "Bad Page State". Also upgraded to final BIOS firmware level with no improvement. Ran memtest86+ for 37 minutes. Will let it run overnight.

Overnight run of memtest86+ 1.07 on the affected HP Proliant DL145 G2 shows no errors after more than six passes.

I plan on trying the proposed kernel
https://edge.launchpad.net/ubuntu/+source/linux/2.6.32-25.43

and the Maverick backport

While running the 2.6.32-25.43 kernel, circumstances which provoked the "Bad Page State" bug failed to do so.

This doesn't really mean that the issue is fixed but it's a hopeful sign.

I'm continuing to test.

Attached is the "lspci -vvnn"

Based on https://patchwork.kernel.org/patch/120327/

Will try "nopat" kernel option and look for PAT in the dmesg.

With PAT on it looks like:

Sep 2 14:35:55 zirconium kernel: [ 0.000000] x86 PAT enabled: cpu 0, old 0x7040600070406, new 0x7010600070106

So far no more "Bad page state" errors.
