First an upgrade from 9 and then a full complete install from ISO then updated.

Expected Behavior:

Mouse should be able to determine focus based on where I click or based on what window is directly under it.

Current Behavior:
-------------------------

Mouse focus sticks to last focused item until Assistive Technologies is turned on or off after any reboot of the system.

If AT is on, turning it off fixes the problem. If AT is currently off, turning it on fixes the problem.

Until this is done, mouse focus can only be changed by Right Clicking the window that currently has focus (opening the context menu) then left clicking onto the window that you wish to have focus.

Windows that have no context menu will hold onto mouse focus until exited, then the gnome panel will gain focus.

This PC does not have any PS2 slots nor do I have a PS2 mouse, or any other mice, to test.

Keyboard Focus and Mouse focus can be and are often separate from each other.

I also am running 10.04 and have test several mice and keyboards only to discover that I am able to regain normal use of the mouse and alt-tab as described by switching the AT setting. I checked and I don't even have the programs referenced from AT installed on my system. Is there something else that could be uninstalled in order to resolve the issue?

I can not reproduce this issue using Ubuntu 10.10. Focus follows the mouse, simply by left-clicking to focus the window. Using the screen-reader, every window I left-click receives focus. Perhaps you can tell me what to set in System -> Preferences -> Assistive Technologies to reproduce your issue?

Ubuntu 10.10 with Logitech LX-6 USB optical mouse. Without doing any upgrades suddenly nearly exactly the same problem happened to me. After doing all the upgrades the problem remains the same. The only difference is that switching Asistive Technologies on/off doesn't help. And Asistive Technologies setting are all default. So I don't think it is something to do with AT.

Sorry, it is exactly the same problem, turning AT on (if it was off)/off (if it was on) help, I just need to click "Close and Log out" instead of "Close".

I can also confirm that this bug also affects my Ubuntu installation, and it happens in both 9.10 (Karmic) and 10.04 (Lucid). I was running a Karmic installation until earlier in the week, and had been for nearly a year, but this issue started to appear in the last week. At around the same point as the problem started to occur, update manager started failing with an "unable to grab the mouse" error message when I attempted to update (I think there may be other bug reports regarding this issue - I don't know for sure that it's related).

With one application window open, things seem to be fine, but as soon as a second application window opens (within the same or another application), I'm not able to grab window / button focus, or move/resize windows with the mouse.

As with the issue Tamas reports above, if I switch the state of assistive technologies from on->off or off->on (depending on the current state), and then "close and log out", the problem goes away when I re-logon to Gnome.

Performed an upgrade to Lucid, but the problem is still occurring when I first boot up the system.

I'm using a Logitech wireless USB mouse on this machine at the moment.

I am also having this same issue. It was also present during the installation off of the desktop image for 10.10. The only way to fix it is by turning AT on or off.

I don't remember how I resolved the issue but this problem went away for awhile. now it's back after a recent update. Same symptoms as before, can't switch window focus with alt-tab etc. Oddly the issue began while I was doing normal work after hours of making no changes to the system.

yes, i have the exact same problem and have had it on ubuntu 9, 10, and 11. i have the exact same mouse.
