1st monitor update missing after period of unresponsiveness

In R3.14.5 the client library was changed to disconnect
channels but not the circuit when a circuit becomes
unresponsive. To avoid introducing load on beleaguered
servers the code fetched updates only for subscriptions
that changed during a period of unresponsiveness. However,
this resulted in a behavior change for
applications expecting a subscription update for
every subscription that reconnects.

Fixed in R3.14.7 by fetching updates for all subscriptions when
switching from unresponsive to responsive. This is nevertheless
far less load when reconnecting channels on a saturated server
compared to R3.14.4.
