makeBaseApp.pl -i should ask for app name, not just use IOC name

There are several places in an st.cmd file that need the name of the previously installed xxxApp rather than the IOC (for example the dbd filename, etc. We're currently just using the IOC name there with a comment in st.cmd that this might need changing manually, but there's no reason not to allow the user to set that name, either from a command-line option or if not given by asking for it.

Janet has implemented this, I'm currently cleaning up the messages to make them a little more friendly.
