Have you re-tried this on Lucid with all recent updates to apt and other systems installed?

I just used the exact same command on Lucid i386, '$ sudo apt-cdrom add -d /cdrom/ -m -a' and I got an entry to /etc/apt/sources.list. Can you run an strace to see where the process is stuck?

strace done, log attached.

sudo strace -Ff -tt sudo apt-cdrom add -d /cdrom -m 0a 2>&1 | tee strace-apt-cdrom.log

FYI, adding a CD to the sources list is successful using Software Sources in Administration menu.
