Proxies: add ability to delete a proxy

Bug Description
Currently there is no way to delete a proxy. This functionality is desirable.
Currently only Admins can list proxies because they have the "proxy" permission. Users with the "proxy" permission should be able to delete proxies.
Note: Proxy id's may be referenced by other records in other tables. Make sure these are properly handled.
