After resume from suspend with encrypted USB drive connected, cannot reattach device

Binary package hint: cryptsetup
I connect my USB Drive with encrypted content (using cryptsetup). After suspending and resuming, a window appears asking for the password, which I reenter. Following correct entry of the password, a message box appears with the following message: "Unable to mount 250 GB Encrypted Error unlocking device: cryptsetup exited with exit code 239: Command failed: Device already exists"
Sure enough, nautilus has two entries: one from before the suspend action which is no longer labeled as encrypted, and, oddly, is now mounted as read-only. The file are accessible through this entry. The other quasi duplicate entry is labeled as encrypted but cannot be mounted (see error msg above).

cryptsetup doesn't spawn any windows prompting for passwords. What software is creating this window and calling cryptsetup? I don't think anything in the default GNOME desktop does this.
