Hi,
I'm not sure which package would be causing this issue -

Symptoms: I can read and burn dvds consistently on my machines internal drive without an issue, but after a period, the discs stop being recognised, and dmesg spits out the following error when a blank dvd is inserted:
$ dmesg

At this point, if I plug in an external dvd-burner and insert the blank dvd into it, the same error appears for this new drive:

which implies that the entire optical device subsystem has gotten itself in a tangle.

Rebooting allows both the drives to operate as per normal again - the period it took for this failure was 6 days of constant uptime (no suspend/hibernate), burning ~5 DVDs a day. Prior to this it was happening every 2-3 days under the same conditions, so the period for failure doesn't seem to be consistent.

As a follow up, I've had this issue crop up 3 times today already (necessitating a reboot each time to resolve)

Hi cleary,

This bug was reported a while ago and there hasn't been any activity in it recently. We were wondering if this is still an issue? Can you try with the latest development release of Ubuntu? ISO CD images are available from http://cdimage.ubuntu.com/releases/ .

If it remains an issue, could you run the following command from a Terminal (Applications->Accessories->Terminal). It will automatically gather and attach updated debug information to this report.

apport-collect -p linux 499343

Also, if you could test the latest upstream kernel available that would be great. It will allow additional upstream developers to examine the issue. Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Once you've tested the upstream kernel, please remove the 'needs-upstream-testing' tag. This can be done by clicking on the yellow pencil icon next to the tag located at the bottom of the bug description and deleting the 'needs-upstream-testing' text. Please let us know your results.
