Default worksheet

The default install of IVLE needs to have a 'sample' subject with a single
worksheet.

This worksheet is not a Python tutorial; it's an IVLE tutorial. It walks
you through IVLE worksheets, for the purpose of selling academics on the
system.

This is the only worksheet someone will see when they 'apt-get install ivle'.

I don't think the default installation should have this, but it should be just a
couple of clicks away once we support worksheet+exercise packages.

I think I'll sit about half way on this - it should be a config option ('Do you want
to install the default worksheet?') and then whichever way you answer, it should be
fairly easy to install or remove it.

Fixed ages ago (sample data is well and truly working, including a default worksheet).
